In order to increase the efficiency of your office visit, we have posted our standard office forms online in a *.PDF format as well as a medical records request form to be sent to your referring physician's office.
Patients may print out these forms (no two-sided printing, please) and complete them prior to the office visit. Please note, these forms will not be accepted electronically.
**Please review our Practice Policies and Procedures, Patient Rights and Responsibilities, Understanding Your Insurance Coverage, Financial Responsibilities, and Privacy Notice sections before signing these forms. These documents are available on the Office Policies and Procedures page. A printed version of the web content is available in our practice brochure which you may request at the time of your office visit.

1. Download Patient Registration Paperwork
Please review the following options carefully to determine which packet of information you need to complete for your type of consultation.
1. If you are a new patient to our practice, have not been seen in the last 2 years and are NOT having a breast related consultation, or were seen by one of our physicians at the hospital for emergency surgery please download and complete the New Patient Packet. Patients scheduling a colonoscopy should also complete this paperwork.
2. If you are a new patient to our practice or have not been seen in the last 2 years and are having a breast, chest wall, or axillary region consultation please download and complete the New Breast Patient Packet.
2. Request Records be sent TO Foxhall Surgical Associates, P.C for your Upcoming Appointment
We ask that you bring all necessary films and reports to your appointment if applicable. We have prepared a standard medical records release form (below) which you can complete and submit to your doctor's office to request this information be sent to you or directly to Foxhall Surgical Associates, P.C.. Please remember, you should contact your doctor's office to determine their policies and procedures for making a medical records request including time frame and associated fees.
